Your Privacy

Each time you visit a hospital, doctor or other healthcare provider, a record of your visit is made. Typically, this record contains your symptoms, examination and test results, diagnoses, treatment, a plan for future care or treatment and billing-related information. ZCH must keep this medical information confidential and you are entitled to a copy of our Privacy Practices (446 KB, PDF), which describes how information in your medical record may be used and how you can access this information. This notice applies to all of your records generated by the facility.

Hospital telephone operators and information desk staff will tell visitors/callers only that you are in the hospital and your room number. ZCH recommends that you appoint a designated spokesperson who will be the person entitled to receive information from your care team about your health.

If you have special needs for privacy, you may request that information not be released. If you believe that your privacy rights have been violated, please contact our privacy officer at 616.772.7556.
